Nebraska State Immunization Information System (NESIIS)


NESIIS is a secure, web-based System that allows health care providers to keep track of immunizations.


To log into your immunization record, you will need the following information about the person's record you are looking up:

  • Name
  • Birth date
  • Social Security number

  • Haemophilus influenzae type b (Hib)
  • Hepatitis A
  • Hepatitis B
  • ​Human Papillomavirus (HPV)
  • Influenza (flu)
  • Measles
  • Meningococcal disease

​​Medicaid VFC Vaccines
For children (age 0-18) who have Medicaid

Walk-ins welcome at most clinics. Please bring your Nebraska Medicaid card, along with your Medicaid Managed Care insurance card (i.e. your Arbor Health, CoventryCares or UHC Community Plan card). CNCAP will need to verify the client’s current Medicaid eligibility prior to administering any vaccines.
